As a new patient to our practice, we appreciate you choosing In Step Behavioral Health. To speed up the registration process, we have provided our new patient intake forms for you to download and complete prior to your appointment. Please bring the completed forms with you 20 minutes prior to your scheduled visit. By filling out these forms ahead of time you will save significant time during your appointment.

Prescriptions
Several days before your prescription runs out please ask your pharmacy to fax a prescription request to (630) 262-2645. Written prescriptions must either be picked up during business hours, or you may send in a self addressed, stamped envelope. Please allow two business days for all refills. There is a $10 rewrite charge for written prescriptions that are lost or not filled before they expire. Routine prescription refills will only be completed during normal business hours, so please make sure you have adequate medication for vacations, holidays or weekends.

FMLA and disability forms require 7-10 days to complete. There is a $45 charge due at time of pick up or prior to submission to recipient.
Non-routine letters can take up to five business days to complete. There is a $25 charge due at time of pick up or prior to submission to recipient.

After Hours Charges
After hours messages sent to Dr. Srinivas Rao, Martin Mika, APN, Pollyanna Vujovic, MSN, FNP, PHMP-BC, Amy Drescher-Crumpley, APN will be responded to generally within one hour and may incur a fee. Here are a few examples you may want to consider before sending a message:
As always, for life threatening situations or emergencies call 911 or visit the nearest Emergency Room.

- Suspected side effects of a new medication or medication change are, in most cases, assessed without cost.
- Extended conversations may lead to a telephone consult charge.

We require 24- hour notice for cancellations and less than that is considered a late cancel. Our office policy is to charge for late cancels and no shows. You can call to cancel or reschedule during our regular business hours or you can leave a message with our answering service.
Urgent prescription refills are filled at the discretion of the prescriber and are not guaranteed. You may be charged an additional fee. If you need a rewrite of a written prescription there is a charge of $10.

We accept cash, personal checks and Visa and Master Card. Payment is due at the time of service unless other arrangements have been made. Payment is considered to be part of your treatment, so please help us to serve you better by paying your bill on time. There is a $40 fee for all returned checks. Co pays are due at the time of service.
Please call our Billing Department at (630) 443-4664 regarding questions or concerns you may have about your bill. They will be happy to assist you with such things as clarification of a bill you received, a payment plan you wish to establish or other billing issues you may have.

We accept assignment of insurance benefits only if we are contracted with your insurance company. You are responsible for your co-payments, deductible and any amount not covered by your insurance. If we are not a provider in your insurance company we ask that you pay, in full, at the time of service. It is the patient's responsibility to contact their insurance company to check on coverage and to obtain authorizations if necessary. Please provide us with any changes in your insurance or address, so we may successfully bill your insurance.
