Appointments
Please call (630) 262-2640 to schedule your first appointment. If our receptionist is unable to take the call, please leave your name, some good times to reach you and your phone number. An intake coordinator will call you back and ask some simple questions and schedule your appointment.
Please note that it is your responsibility to obtain prior authorization from your insurance carrier if it is required.
Once you schedule your appointment, we ask that you arrive 15 minutes before your appointment with your completed registration paperwork, photo ID, and your insurance card.
Additional important points:
Getting the most from your visit with the care provider. While bringing the completed forms with you can save time in our waiting room, mailing it to us ahead of your appointment will enable your care provider to review the information prior to seeing you.
ISBH requires 24-hour notice for cancellation of your appointment. A new patient who is a No Show for the first visit will be charged in full. This fee is not insurance reimbursable.
All No Show or Late Cancellations will result in the following fees:
Late Arrivals. Please notify ISBH if you will arrive more than 10 minutes late for an appointment.
Messages may be left with our answering service after business hours (listed below). Due to HIPAA regulations, with the exception of speaking with parents or guardians of minors or an intellectually impaired patient, our office staff will only talk to the patient directly about appointment and billing issues.

- In most cases you do not need a referral to set up an appointment. However, if your insurance plan requires this, make sure that you call your primary care physician before you set up your first appointment.
- We accept a variety of insurance plans, Medicare and self-pay. It is important to check with your insurance provider to make sure that we participate in your insurance plan if you want us to bill that company directly.
- Our office staff will contact you to confirm the first appointment one to three days before your appointment.

- $110 fee for medication management visits
- $140 fee for Psychotherapy visits

- Late arrivals for psychotherapy may reduce your session time.
