- Demographic information such as address, email, date of birth, phone number
- Are you seeking an appointment with a specific Clinician? To see the current clinicians' bios at In Step please click here.
- How did you hear about In Step?
- The name of your primary care physician.
- If you are insured, please make your insurance card accessible.
- Reasons for seeking treatment.
- In Step offers in-office (in person) as well telehealth (online) appointments. What type of appointment would you prefer?
- Billing information such as credit card information. Please note that prior to schedule an appointment, a $50 new patient's fee will be collected and credited to your account.

- Are you coming for a hospitals or step-down unit?
- Have you ever been hospitalized during the past year? If yes, please provide us with the reason.
- Are you currently with a Therapist? If yes, please provide us with his/her name.
- Are you currently on any psychotropic medication? If yes, please list your medications.
